Clay County Vital Records

Ceritified copies of birth or death records of anyone born or deceased in Clay County, Illinois may be obtained from Flora City Clerk's office located at Flora City Hall, 131 E. Second St., Flora IL 62839, Monday - Friday, 9 a.m. - 4:30 p.m. (618) 662-8313.  For your convenience, application forms for birth and death certificates are available below.  Fees for certificates are as follows:

• Birth Certificates

       First Copy - $10.00
       Each Additional - $3.00

• Death Certificates

       First Copy - $14.00
       Each Additional - $6.00

Out of town/state requests may be mailed to City of Flora, PO Box 249, Flora IL 62839, along with application form, copy of the individual requesting driver's license and fee.
